Notified by nursing that despite knowing Dtap, Hib, PCV20 were expected/ordered to be given, wrong vaccine given as Vaxelis (Dtap/Hib/IPV/HepB) instead of Dtap. Child thus received extra dose of Hib/HepB/IPV.

Current illness

Documented at 15mo check-up 6/5/26- Diarrhea non bloody x 1 week, loose non bloody stools, mom thinks may be getting a little better/improving. No vomiting. No fever, no rash.
