Symptom narrative

Client reports flu like symptoms on April 21, 22 and 23rd. On Thursday, April 24, 2026 he developed sores in his mouth and thought he had thrush. On Monday, April 27, 2026 he was seen at Health Center Same Day Clinic and was given Mouthwash to numb his mouth so he can eat. He was told it wasn't thrush. He was also told by the provider he saw that this was a vaccine reaction and to have the health department report it. On Monday May 5th, 2026, he saw a dentist who refilled the Mouthwash prescription since the sores were still present although they were better. Client reports that the sores were gone completely by May 11, 2026. He does not want the second dose in the series.
