This case involves a patient of unknown age and unknown gender who had received YELLOW FEVER VACCINE - (YF-VAX) was diluted with sterile water rather than supply diluent with no ae (adverse event) The patient's past medical history, medical treatment(s), vaccination(s), and family history were not provided. On an unknown date, the patient received a dose (dose: unknown, frequency: unknown) of YELLOW FEVER VACCINE - [YF-VAX] solution for injection (batch/lot number: unknown, expiry date: unknown) (strength: unknown) via unknown route in an unknown administration site for immunization, due to administration of vaccine diluted with sterile water. YF-VAX was diluted with sterile water rather than supply diluent with no ae (product preparation issue, onset date: unknown, latency: SAME DAY). Action taken: Not applicable.
