missed/late dose; This non-serious case was reported by a nurse via call center representative and described the occurrence of incomplete course of vaccination in a 7-year-old female patient who did not receive DTPa-HBV-IPV (Pediarix) suspension for injection in pre-filled syringe for prophylaxis. Previously administered products included Pediarix (received first dose on an unknown date) and Pediarix (received second dose on an unknown date). The patient did not receive the 3rd dose of Pediarix. The patient experienced incomplete course of vaccination (Verbatim: missed/late dose). The outcome of the incomplete course of vaccination was not applicable. This report is made by GSK without prejudice and does not imply any admission or liability for the incident or its consequences. Additional Information: GSK Receipt Date: 04-JUN-2026 The nurse reported that patient had missed a DTaP dose and the last dose had been Pediarix but that the patient had still been deficient by one dose. The nurse had wanted to know if the child could receive Boostrix at that time. The batch number was not provided and a request for this information has been made.
