Symptom narrative

Pt reports feeling dizzy after Men ACWY, Men B and HPV administration and started to vomit about 2 mins after injections. Pt diaphoretic and reports she is hot. Pt remained alert and oriented during entire visit. Syncope episode without injury noted. Wet towel placed across pt forehead. Cup of water and snack provided. Vitals monitored and documented x3 sets. Pt observed for 30 minutes post immunization administration. Pt states ?I feel fine. I?m ready to leave now.? Pt denies any dizziness or feeling faint. Pt stable and ambulatory at departure and denies any syncope symptoms.
