On 05-JUN-2026, the patient received the 1st dose of Shingrix. On 05-JUN-2026, an unknown time after receiving Shingrix, the patient experienced underdose (Verbatim: Potential lower than recommended dose/ patient may have received 65 percent of the Shingrix vaccine but unsure of exact amount). On an unknown date, the patient experienced product leakage (Verbatim: leaking at injection site following injection). The outcome of the underdose and product leakage were not applicable.
