administration of only the liquid component of Penmenvy; so they only got the syringe and not the vial; This non-serious case was reported by a nurse via call center representative and described the occurrence of inappropriate preparation of medication in a 16-year-old male patient who received Men ABCWY NVS (Penmenvy) suspension for injection in pre-filled syringe (batch number 7J5LF with expiry date 07-JUL-2027) for prophylaxis. On 05-JUN-2026, the patient received Penmenvy. On 05-JUN-2026, an unknown time after receiving Penmenvy, the patient experienced inappropriate preparation of medication (Verbatim: administration of only the liquid component of Penmenvy) and inappropriate dose of vaccine administered (Verbatim: so they only got the syringe and not the vial).The outcome of the inappropriate preparation of medication and inappropriate dose of vaccine administered were not applicable.
