5/28/26 per mom Fever of 102 did Tylenol and ibuprofen every four hours. fever never went below 100. 6/8/25 mom called " Parent of the patient calling for her son who has been to the UC and ER for a fever. Last night he developed diarrhea too. His current temp was 103F, rectal. Mom gave Motrin and now his temp is 100F, rectal. He is still feeding but not as well as he normally would. He is having wet diapers but not as frequently, not going over about 4hrs without having a wet diaper. He has had about 6 episodes of diarrhea in the last 24 hrs. He has no vomiting. Mom isn't sure if he is in pain at all. He is uncomfortable, not himself, and very fussy. His stool has no blood, dark green in color. He is moving his arms and legs, sitting up, and playing with toys. He is on Augmentin and nystatin for a sinus infection. Mom thinks the vaccines he has received tested. He has had a horrible reaction to the vaccines and not gotten better. Symptoms started: 3 wks. Patient has tried ibuprofen and Tylenol with mild improvement" submitting VAERS report as mother wants vaccinations "tested" office does not feel that is necessary but shared a VAERS report would be completed
