Patient was seen in the office today for her 4 year physical, which requires her to get her 4 yr vaccines. Appt was at 10 am, wasn?t seen until 10:33am. Patient was examined by PCP, Dr. (name withheld). After being examined, patient was ready for her 4 year vaccines. Two vaccines were given today (Quadracel = DTaP/Polio (EXP: 8/2027) and Proquad =MMR/Varicella (EXP: 3/28/27)). Quadracel and Proquad were administered at 10:49 am. Quadracel given IM in the left deltoid and the Proquad in the right deltoid (IM). Patient cried at the time of vaccine administration but was able to be consoled by mom. Both patient and mother left the office after the vaccines in good condition. At approximately 11:05 am, mom came back up to the office reporting that Patient stated she felt short of breath and felt chest tightness. During this time, she vomited in the waiting area x 1. Mom and Patient were brought back to an exam room where 3 nurses (names withheld) and MD (name withheld) were present to examine patient. A pulse ox was put on patient?s finger, at 100% with a HR of 157. Nurse assessed pts lungs and lungs were clear to auscultation. Patient reported having nausea, but did not vomit, however, was dry heaving. Still c/o chest tightness. Dr (name withheld) examined the patient by assessing her lungs and lungs clear on auscultation. She was able to swallow, breath, and speak. Still upset based on facial expression, but mom able to console child. At 11:07 am, 5 mL of Zyrtec administered via oral route. At 11:07, Epipen was administered based on pt?s weight (37 lb) ? 0.15 mL in the left thigh. Oxygen was administered at 11:08 at 6 L/min. Continuous monitoring of pulse ox, at 100%, HR continued to be elevated in the 130?s. At 11:11am, ambulance was called. While waiting for ambulance, continued with monitoring patient?s O2 saturation and HR. Vitals taken at 11:16 am, BP:90/54 (L arm), O2: 100%, HR :138, RR: 28. At 11:20 oxygen was removed per Dr?s orders, O2 at 97% RA. At 11:24 am Ambulance arrived. Report given by (name withheld) RN. Pt assessed by EMT. Transfer summary given along with immunization record. Immunizations that were administered were highlighted & explained to EMT?s. Times & dosing of Zyrtec and Epipen written on the transfer summary with pts vitals. Mom walked out with child in her arms with EMTs at 11:30 am.
