Symptom narrative

Approx. 5 minutes after administrating vaccines, patient and family were walking out of clinic. Patient started sweating, became pale, said she was seeing black spots and felt as if she was going to pass out. Patient sat down on the floor. Nursing staff took patient in a room to lay on the exam table. Patient was offered water and a cool rag. Patient recovered quickly.
