Medical assistant did not reconstitute PenMenvy and only administered menB component; Medical assistant did not reconstitute PenMenvy and only administered menB component; This non-serious case was reported by a nurse via sales rep and described the occurrence of inappropriate preparation of medication in a adult patient who received Men ABCWY NVS (Penmenvy) suspension for injection in pre-filled syringe for prophylaxis. On an unknown date, the patient received the 1st dose of Penmenvy. On an unknown date, an unknown time after receiving Penmenvy, the patient experienced inappropriate preparation of medication (Verbatim: Medical assistant did not reconstitute PenMenvy and only administered menB component) and inappropriate dose of vaccine administered (Verbatim: Medical assistant did not reconstitute PenMenvy and only administered menB component).
