She ended up passing away 5 days after vaccine; pain on arm that she got vaccine shot in; feeling awkward/change in her demeanor, she was very quiet/very abnormal for her; assumed was blood clot; This is a spontaneous report received from a Consumer or other non HCP. A 76-year-old female patient received pneumococcal 20-val conj vac (dipht CRM197 protein) (PREVNAR 20), on 22May2026 as dose number unknown, single (Batch/Lot number: unknown) at the age of 76 years, in arm for immunisation. The patient's relevant medical history included: "diabetic" (unspecified if ongoing); "kidney issues" (unspecified if ongoing); "high blood pressure" (unspecified if ongoing); "gout" (unspecified if ongoing); "vein popped out" (unspecified if ongoing), notes: vein where she had blood drawn. Concomitant medication(s) included: FUROSEMIDE oral; ASPIRIN EC oral; SPIRONOLACTONE oral; HYDRALAZINE oral; LISINOPRIL oral; FENOFIBRATE oral; ATORVASTIN oral; JARDIANCE oral; ALLOPURINOL oral; AMLODIPINE BESYLATE oral; OZEMPIC. The following information was reported: THROMBOSIS (medically significant) with onset May2026, outcome "unknown", described as "assumed was blood clot"; FEELING ABNORMAL (non-serious) with onset 22May2026, outcome "unknown", described as "feeling awkward/change in her demeanor, she was very quiet/very abnormal for her"; VACCINATION SITE PAIN (non-serious) with onset 22May2026, outcome "unknown", described as "pain on arm that she got vaccine shot in"; DEATH (death, medically significant) with onset 27May2026, outcome "fatal", described as "She ended up passing away 5 days after vaccine". The patient underwent the following laboratory tests and procedures: Blood test: (07May2026) Unknown Results. Therapeutic measures were not taken as a result of vaccination site pain, feeling abnormal. The patient date of death was 27May2026. The reported cause of death was unknown. No autopsy was performed. Clinical course: Patient started to feel awkward within hour or 2 of vaccine. She ended up passing away 5 days after vaccine. Her symptoms started right after. From doctor's office, wen to withheld, noticed change in demeanor, she very quiet, in pain. Does not know because, she was very quiet, patient was never quiet. Whole ride home, patient was quiet, was very abnormal for her. When they came home, patient started having pain on arm that she got vaccine shot in. Was bad enough to where, his Dad had to give her massage. That lasted a day. After that, noticed, she could not even pick up a frying pan. Was in pain that continued until she passed away. Did notice, patient had blood work drawn on her hand, maybe a week and a half prior. The vein where she had blood drawn, noticed like, a vein popped out, was like, sticking out. Noticed the next day, he believes the next day or day after, when she passed away. Medical Conditions: Know she was diabetic, had some kidney issues, had high blood pressure, had gout, but when he went to go see doctor the next morning, when all this happened, he looked at his records, was actually shocked, said she had all those issues but, had those issues for years. He did not see anything that could have caused this because, she was very healthy, besides those issues she had for years, was very healthy. Test Results: Knows she had blood work, done, 07May2026. Clarified, when caller previously referred to patient feeling awkward, within an hour or 2 after taking vaccine, was referring to later statement that he noticed change in her demeanor, she was very quiet and in pain. Patient would try to get up from table, try to leave. She does not drive, does not like to go outside, was not herself at that time. Caller thought she argued with his Dad and missed it. There was no argument, nothing there, was just her. The ride home is maybe 30 minutes or so, she was very quiet in car, was never quiet at all. Was very quiet until they started to get closer to house, patient started to have pain. Pain: Clarified, patient started having pain that day, in arm she had vaccine shot in. Was bad enough to where his dad had to give her massage. Does not know if pain was a little lower or higher, where she had the shot but, patient kept saying, pain was where she had the shot, the vaccine. Unknown to caller, if it was right or left arm that she had shot in, that was in pain. Clarified, as previously states, was the next day patient could not pick up the frying pan. Pain lasted the 5 days up until the day she passed. Passed way complaining about the pain. Treatment for pain: No. Date of death: Clarified as 27May2026. Cause of death: Unknown. Death certificate does not have that all. All of patient's kids and caller's Dad, really wanted to get autopsy because, they don't' know what happened. Went back and forth with patient's primary doctor, he had written down, the same night she passed, that it was blood clot. Then, next morning when they went over there, caller started to remember a little thing that happened when patient was being quiet after taking the vaccine. Doctor then changed it to known. Asked for state or county, to see if they would approve for the autopsy and, day or 2 days later, found out, they did no approve for autopsy. They have low income, so, would cost dollar 7,000 to get autopsy. Was very hard to leave that because, they just know she was healthy, was good before the vaccine. Then, doctor mentioned he was going to leave blood clot on there because, caller guesses, the paramedics came and put something down her throat, pulled it out, was blood.So, assumed was blood clot. Caller wrote doctor a letter, the day they had to decide to create or have burial. Wrote that he remembered about the vein, pretty much explained he believed it was the vaccine that caused her to pass, because of everything he noticed. Was all the little things, as far as her having pain, being quiet. Clarified, was after the paramedics put something down patient's throat, pulled out, noticed blood, which led to assuming blood clot. Doctor changed it because he knew of blood clot but, did not know that patient had the vaccine until caller explained that, she got the vaccine 4 days prior to passing. Explained everything he just explained to the doctor. That is when they tried to get autopsy but, state, county, whoever approves it, would not approve it. Would have to come out of pocket, to pay for autopsy. The patient did not receive any other vaccines on the same date as the Pfizer vaccine. The patient did not receive any other vaccines within 4 weeks prior to the first administration of the suspect vaccine. No AE required a visit to the Emergency Room or a Physician's Office. The patient had not previously receive the Pfizer suspect vaccine.
