Symptom narrative

After receiving MMR vaccine (and HepB) from pharmacist intern, patient stated she was dizzy and felt nervous. After about 45 seconds the patient stated she was going to pass out. Patient passed out for approx. 1 minute then seized as she woke. Patient stated that she has never passed out from vaccine previously, however has has episodes of passing out in the past and has had seizures in the past when waking from this. Offered to call additional medical care, patient declined. Offered water, patient accepted and waited for another 10 minutes for monitoring. Patient stated she felt fine and walked out without problems.
