Patient was seen 6/10/2026 at 8:50 for routine vaccination of Menveo, Tdap and HPV. Vaccines were administered and immediately following HPV vaccination the patient had a syncopal episode. Patient was advised to lay down after he came to 20 or so seconds later. He is feet her elevated and blood pressure taken. BP was 118/72. Dr came in and educated the patient and his step father on this common side effect. Patient was given juice and snacks and advised to stay in that position for 5 minutes then slowly sit up. Patient tried sitting up 5 minutes following instruction and had another syncopal episode. He was advised to lay down for 30 minutes per Dr and Dr requested that I get orthostatics and an EKG after 20 minutes. After about 20 minutes patient's color had returned to his face but B/P was only 83/49 when taken. He was sat up to attempt a sitting B/P and Pulse and started to loose color and felt dizzy. He was then advised to lay back down. Dr was consulted and advised the patient to go to the ER.
