Symptom narrative

Patient came in 3 days after receiving the vaccine to report redness and itchiness at the site of administration. This had begun approximately 24 hours after receiving the vaccine. Patient reported no warmth, swelling, tenderness in the area. She had tried some topical hydrocortisone cream to help alleviate the itching, but it was not completely helpful.
