Symptom narrative

After the vaccine I had massive pain in lower back. I took Tylenol w/ codeine but nothing I took would help with the pain. The heating pad did not help, and I couldn't stand up straight and could barely walk. I could not get comfortable. The pain was excruciating for a couple of days and is finally starting to ease up some.

Medical history

diabetic, GERD, asthma, IGG4, synostosis of the spine, cholesterol, HBP, occasional kidney stones,

insulin, amlodipine, Celebrex, zolaft, levetiracetam, losartan, omeprazole, tamsulosin, tramadol, MiraLAX, B12, Vitamin C and D, Cpap, Tylenol 2 prn, Rituxan infusion,

Allergies

reactions to medications, ie, metformin, lilac
