Symptom narrative

Patient got RSV vaccine twice at an offsite clinic, it was not caught until after the clinic while processing the vaccine at the store. The Pharmacist who administered the vaccine at the clinic site (Not reporter) was not aware the patient already got the vaccine; reporter caught it later on at the store while processing the vaccine
