Nurse confirmed that there were no symptomatic events. No additional AE; female 4 month old infant was mistakenly administered VAQTA; This spontaneous report was received from a nurse (nurse prescriber) and referred to a 4-months-old female patient. The patient's pertinent medical history, concurrent conditions, drug reactions or allergies, and concomitant therapies were not reported. On 21-May-2026, the patient was mistakenly vaccinated with Hepatitis A Vaccine, Inactivated (VAQTA) 0.5 mL, total, Lot Number: Z013008 (Expiration date: 17-Oct-2026) (route of administration and dose number in series were not reported) as prophylaxis (Product administered to patient of inappropriate age). It was confirmed that there were no symptomatic events. No additional adverse event (No adverse event).
