Pt presented to pharmacy with red rash distal to deltoid injection site 6/13/26. Rash was pronounced, had started spreading down arm towards elbow per pt since Thursday, but was similar today vs yesterday. Pt reports rash is itchy, warm, and slightly sore. Pt also reports Weds PM she experienced fever, chills, fatigue, and small knot, which have since subsided. Pt reports she applied hydrocortisone and benadryl topical at home, which provided some relief. (RPh on duty) directed pt to urgent care in case she requires antibiotic or steroid in case of possible infection. Pt disclosed vaccine administrator cleaned the area well prior to injection with alcohol.
