Patient called the pharmacy at 2:40pm to inform the pharmacy that she felt the site of her recent Shingrix vaccine was too high in comparison to her previous dose. She stated that she couldn't move her arm and that she was in a lot of pain, rating at a 9 out of 10. The pharmacist advised that the patient use ice and acetaminophen if appropriate to help with the injection site pain as well as gentle movement of the arm to keep it limber. The pharmacist also gave the patient the option to return to the store to verify the correctness of the placement of the injection if she was so inclined. The patient returned to the pharmacy later that afternoon and the pharmacist verified that the placement of the injection was correct, approximately 2.5-3 fingers width down from the acromion process, covered with a dot band-aid. The patient reiterated that she was still in a great deal of pain and stated that she could not move her arm. The pharmacist explained that the second dose can cause more discomfort, that pain or mild swelling at the injection site was common, and that using ice and trying to keep the arm mobile and limber usually helped lessen overall discomfort. The patient denied using any ice after the receiving the injection and stated that she could not take acetaminophen due to poor liver status. The pharmacist again recommended the use of ice and gentle movement but also advised patient to seek medical attention if she felt the home treatment recommendations were not sufficient to alleviate her discomfort. The patient then decided to purchase a package of instant ice packs and reusable gel ice pack and left the store.
