Symptom narrative

Patient received yellow fever vaccine within the past 2 weeks. She came in today with complaints of abdominal pain, nausea, vomiting. Her LFTs are extremely elevated concerning for hepatitis. Pharmacy mentioned possibility for vaccine associated viscerotropic disease and recommended filling this out. I do not have the vaccine information as it was given at a different facility I do not have access to that information. The patient is currently being admitted to the hospital for further observation
