did not mix the vial with syringe; did not mix the vial with syringe; This non-serious case was reported by a consumer via call center representative and described the occurrence of inappropriate preparation of medication in a patient who received Men ABCWY NVS (Penmenvy) suspension for injection in pre-filled syringe for prophylaxis. On 08-JUN-2026, the patient received Penmenvy. On 08-JUN-2026, an unknown time after receiving Penmenvy, the patient experienced inappropriate preparation of medication (Verbatim: did not mix the vial with syringe) and inappropriate dose of vaccine administered (Verbatim: did not mix the vial with syringe). The outcome of the inappropriate preparation of medication and inappropriate dose of vaccine administered were not applicable.
