Symptom narrative

Woke up 6/12/26 with patient reporting that she woke up this morning not feeling well. She describes a not feeling well as generalized fatigue. She denies any cough, sore throat, or chest pain. She had associated shortness of breath this morning. EMS was called. She was found to have SpO2 in the 90% and was started on 1 L of supplemental oxygen. Per staff at the nursing home, patient had reported shortness of breath yesterday and was given a dose of Lasix to see if this would help her symptoms but they did not.

Current illness

Hospitalized 5/28-6/1 with with epistaxis and subsequent generalized weakness and presyncopal episode. She was found to have critical aortic stenosis, acute blood loss anemia, dehydration, possible UTI, hypomagnesemia.
