Symptom narrative

Patient passed out ~ 2 minutes after vaccine administration. Patient had a long term history of passing out during injections in the past. He recieved the injection, said he felt fine, but I then mentioned he felt light headed. I suggested sitting in the consult room with his mom for a few minutes until he felt better. When I went to gather documentation for his school regarding the vaccine, he passed out. His mom tried to get him to regain consciousness, but after about 20 seconds I had a bystander call 911. He regained consciousness after about 45 seconds to a minute after he passed out. During that time we relocated him to the floor.
