Symptom narrative

Client notified PHD RN on 6/12/2026 reporting localized redness, swelling, and itching around the injection site of the IPOL vaccine on the left arm. This reaction occurred less than 48 hours following IPOL administration. RN followed up with client on 6/15/2026 and patient stated the site is improved and all symptoms have resolved. Client denied other symptoms including measured fever, or GI upset. Client did state she felt as though she had some chills at night when going to sleep. In reference to above, this RN administered Heplisav-B, Boostrix and IPOL at this visit. Client received Varivax vaccine after this visit at another clinic not connected to Health Department.
