Symptom narrative

pt came back from getting vaccination approximately 30 minutes and was bleeding uncontrollably, bleeding was bleeding through gauze and wrap. Pt advised to seek ER help. At the ER pt was rewrapped and was told to lay down for 30 minutes, bleeding stopped and pt was discharged. She was told to go home drink plenty of water.
