Symptom narrative

There are no known adverse events. Issue was a vaccine error. Patient received Vaxelis today, however he was not due for Vaxelis as they received the 3rd Pentacel on 05/18/2026. Parent was spacing out vaccines and was only due for Vaxneuvance and Rotateq at today's visit. Additionally, parent has been refusing Hep B vaccination. Inadvertently patient was given Vaxelis as well as Vaxneuvance and Rotateq.
