Symptom narrative

Within several hours of receiving vaccination, patient developed severe arm pain and swelling. The pain became so severe that the patient went to the emergency room for treatment. Besides severe pain and swelling, the patient also had a low fever. ER administered morphine and instructed the patient to keep icing the area when he returned home. Upon follow up with patient 6 days later, there is still some pain and swelling but not severe.
