Symptom narrative

Patient was given the 3 vaccines (Bexsero, Menveo, Gardasil) and after about 5-10 minutes after vaccines and PPD were administered, patient said she was feeling dizzy and dropped the cup of water that was offered to her and water splattered on her, the bed and the floor. She was transferred to another clean room and passed out while Dr, staff and her mom were supporting her and she was laid on the floor, her head and neck were protected. No seizure activity was noted. She woke up after 3-5 seconds of syncopal episode and was immediately alert as to time, place and person and continued to say she was better. She got up and was supported into the patient bed and laid down but preferred to sit up and drink water. SHe said she felt better and felt fine, no pain, no dizziness, no breathing concerns, no palpitatins, no weakness. She was given and ate a peach fruit. Her vital signs were stable at 11:15 am, HR=88, regular rate and rythm, RR=20, O2 sat=100% , BP 115/75. the rest of the exam was normal. She continued to normally talk to her mom. She was observed in the office and she left the office at 11:35 am in stable condition and back to her baseline, talking and ambulating normally, unsupported. Mom was given instructions to observe her closely and stay home in 24 hrs and no driving nor strenuous activities for a week. Advised precautions and prevention of repeated episodes. Discussed the need to slowly get up ie from reclining to standing position, sitting up, dangling legs then once adjusted, to stand up slowly and avoid sudden change of position. Advised to drink plenty of fluids, eat on time ,do not skip meals, get enough sleep and rest. Have a companion within reach for precaution. If dizziness, weakness or syncopal symptoms recur, to call EMS and bring to ER
