14 year old patient was given Proquad. Proquad is for patients 12 years and younger. Called patients parents parent reports no symptom, signs, or adverse side effects at the injection site parent reports no fevers, hives, rash, or seizures. Immunization given on 06/10/26 no adverse reaction noted on 6/16/26.
