Symptom narrative

Patient received 2 doses of HPV on left deltoid, intramuscular. The Physician and patient's mother were notified immediately. Physician spoke with the patient, and he informed the physician he felt fine. No symptoms of dizziness, vomit, etc. Vaccine occurred at approximately 9:20 AM on 06/16/2026.
