Triage note from registered nurse: Pt presented to the clinic for a visit w/psych provider. When the MA-R was checking in pt and went to do the BP she noted redness and swelling on the pt's R arm. MA-R came to get me (RN) to evaluate the pt. The pt was added to the RN nurse schedule. I examined the pt's R arm and the pt's R arm was red and warm to touch. I noted the injection site for the vaccine was not in the deltoid but in the lower muscle in the R arm. The redness extended around the pt's arm in an area which was 6x8 in. The pt's arm was marked w/ a purple marker. I examined and palpated the pt's lymph nodes under the arm and there was no additional swelling noted. The pt denied any additional Sx. The pt was given the opportunity to see a provider to access his R arm or to continue his appt w/ psych provider as he could not see both providers in one day. The pt was advised to take 50 mg of benadryl when he went home, to watch the site for increased redness and swelling, and to contact us top come in for an appt if so.
