Symptom narrative

**Adverse Event:** Injection site abscess requiring incision and drainage **Clinical Summary:** Local induration and soreness was report on 05/20/2026, one week after administration. Patient reported that area was gradually improving. Approximately 2.5 weeks post-vaccination (06/01/2026), the patient reported persistent shoulder pain, swelling, erythema, and warmth at the injection site. A picture reviewed at that time showed mild erythema with dependent bruising. The patient was advised to take ibuprofen and apply warm compresses. On 06/04/2026, in-office evaluation revealed a palpable annular area approximately 1.5 centimeters in diameter with induration and fluctuance. No systemic symptoms, lymphadenopathy, fever, or hemodynamic instability were present. **Intervention:** Incision and drainage performed in office. Purulent drainage obtained. Wound culture sent. Area probed for lysis of loculations. Wound covered with bandage and allowed to drain. Empiric trimethoprim-sulfamethoxazole (Bactrim) prescribed for 7 days pending culture results. **Outcome at Time of Report:** Abscess drained with immediate symptom relief. Wound culture was negative for pathogens, indicating a sterile abscess. Counseled antibiotics were not required at this time and could be discontinued. Supportive and wound care counseling provided, including recommended follow-up in 48-72 hours. Patient recovering; no systemic complications identified.
