Symptom narrative

Immediately after administering penmenvy vaccine pt fell backwards on exam table hitting back of head on the wall then had a tonic episode lasting about 5 seconds. Pt never had LOC but seemed out of it. I laid patient down on exam table with legs flexed. BP measured was low 80's/50-60 with HR in the 40's, repeated BP's remained low and HR then decreased to 37. Pt drank 3oz pediasure. Pt was still awake and alert but quiet. EMS was called. Subsequent BP's improved a little and HR increased to 59-64. O2 sat 98% RA. Pt was take via EMS to Hospital.
