Symptom narrative

Received vaccine 6/12/26 on left arm. 5 minutes after vaccine, she describes left upper & lower leg numbness, tingling, and weakness that caused her to stumble. Later on felt same sensation on left arm. Occurs on & off. Seen here 6/12/26 and ED on 6/3/26. See attached ED notes.
