Reported Symptoms: 10047862:WEAKNESS; 10047865: WEAKNESS GENERALIZED; Narrative: Per reporter they stated, "Patient received COVID vaccine booster 11/7/25 by clinic RN. Wife called 11/12/25 and reported fever/aches the day following vaccine, as well as inability to walk more than 3 steps. PT evaluated and noted decreased walking ability/strength following COVID vaccine administration. He has not gotten back to his previous baseline prior to COVID vaccine and was enrolled in hospice 1/16/26 due to advancing CKD and CHF. Neuro to r/o Gullain Barre." Could not find follow up neurology note. RN note including vaccine and name and where it was given but no lot and expiration date or NDC was provided in the note and documentation.
