Symptom narrative

During the post-vaccination observation period, the patient reported feeling faint, lightheaded, and dizzy at approximately 1540. Vital signs obtained at 1546: BP 92/61 mmHg, HR 60 bpm, SpO2 98% on room air, temperature 97.2°F. Patient was assisted into a supine position with legs elevated. Patient remained alert and responsive throughout the event. At approximately 1555, patient reported improvement in symptoms and stated feeling better. Continued observation performed. No further complaints reported.
