pt came in on float schedule at VGB for Tdap and MCV , MA did the query pt record showed not received mcv #2 or tdap , MA consulted w another MA who also noticed pt was due , MA administered tdap on upper LD +mcv lower LD GAVE W/O COMPLICATIONS , later that day MA was re- check visit notes & checked immunizations reconcile popped up , pt is aware of extra dose
