Symptom narrative

About 2 minutes post vaccine administration, the patient became quiet and started leaning back with eyes rolled back and began visibly shaking with seizure like activity. Patients color was pale, lips were pale. Episode lasted less than 10 seconds. Patient woke up after this episode and reported he felt tingly and then his head dipped forward before he passed out and had seizure like activity. Patient was monitored in office for about 25 minutes. During that time, he had periods of tingling in his hands, then feet, and an overall feeling of warmth in his body. Provided Gatorade and graham crackers, cool cloth on his neck. Allowed patient to soak his hair in water. This helped. Patient reported headache, sleepiness and arm pain upon leaving clinic. Provided tylenol as ordered and requested by mom for headache. Encouraged mom to call with any questions. Mom verbalized feeling comfortable leaving with child. Dr. evaluated patient and discussed the situation with mom prior to the patient and mother leaving.
