After client had received YF and typhim vaccines she ambulated to the waiting room with no evidence of distress with her sister (name withheld) and father (name withheld) while this RN completed the vaccinations for pt's mom and other sister. Per client's father, he witnessed client faint while sitting in a chair in the waiting room, which per dad she fell out of and hit her lips and nose on the ground. Client appeared to have some redness in that area of her face and reported some mild soreness. No lacerations noted. Denied other head trauma/pain. Father laid patient down on waiting room floor with her head in his lap and states that she quickly regained consciousness. When RN responded client was already conscious. Gave her ice pack for chest, cold water, and lifted her legs/had her do isometric exercises. BP was 124/64 in left arm. Pupils equal, round, regular and reactive to light and accommodation bilaterally. Pt alert and oriented x3, and denies headache, nausea, confusion. Advised her to tell parents if any of these symptoms do appear, and also spoke with parents about concussion precautions, though it seems unlikely based on where impact occurred. Client stayed laying down with father until she gradually felt well enough to sit and eventually stand. Client's mother later stated that client had fainted from a shot before - the flu shot - but that "it had been years" so she did not think to mention it before vaccination. Client left clinic NAD with both parents and sisters.
