Symptom narrative

6/17/26, patient sent a online chart message to Dr with an attachment picture of her left upper arm with a very large swollen red area with a dark spot mid-center of insertion site and outside margins. She also had taken a black marker and had outline the outside margin and had an additional line out even further from midpoint. Her email said it was hot, swollen, minor itching, few mm of height induration at insertion site. She is self-treating site with icing, Benadryl 25 mg tablet, and ibuprofen for pain at time picture taken 6/17/26-9 am.

Current Outpatient Medications ? cholecalciferol (VITAMIN D3) 50 mcg (2,000 unit) capsule Take by mouth daily. ? psyllium seed-sucrose (METAMUCIL, SUGAR,) oral powder Take by mouth every day- Drink at least 8 ounces (240 mL) of cool

Repeat call on 6/17 at 1650 to inquire status update. Patient had no new symptoms develop but was inquiring about an antibiotic. Dr sent a prescription for Clindimycin 300 mg capsules; To Take 1 capsule (300 mg total) by mouth 3 (three) times a day for 7 days and she will also take Benadryl 50 mg oral the evening of 6/17/26 after she is home from her work.
