Symptom narrative

brought in by ambulance from home with chief complaint of chest pain that radiated to her back which began prior to arrival. Patient endorses that she did receive the shingles vaccine today. She then began to experience a chills sensation and then started to experience chest pain which radiated to her back. Patient denies any history of chronic back pain. States she has never had pain like this before. On EMS arrival was noted to be cool and diaphoretic and tachycardic into the 160s. And route her heart rate improved into the 100s. Denies obtaining a rhythm strip at that time. Patient reports chills but denies fever. Describes chest pain that radiates to her back and her lower right side of her chest. Denies nausea, vomiting, diarrhea, headache. Denies any other complaints at this time. Patient endorses overall concern for shivering and fever.
