AE - Penmenvy - Only diluent administered; AE - Penmenvy - Only diluent administered; This non-serious case was reported by a nurse via call center representative and described the occurrence of inappropriate preparation of medication in a 17-year-old female patient who received Men ABCWY NVS (Penmenvy) suspension for injection in pre-filled syringe (batch number 499J9/B with expiry date 03-APR-2028) for prophylaxis. On 28-MAY-2026, the patient received Penmenvy. On 28-MAY-2026, an unknown time after receiving Penmenvy, the patient experienced inappropriate preparation of medication (Verbatim: AE - Penmenvy - Only diluent administered) and inappropriate dose of vaccine administered (Verbatim: AE - Penmenvy - Only diluent administered). The outcome of the inappropriate preparation of medication and inappropriate dose of vaccine administered were not applicable. This report is made by GSK without prejudice and does not imply any admission or liability for the incident or its consequences. Additional Information: GSK Receipt date: 16-JUN-2026 The nurse reported the inadvertent administration of only the prefilled syringe containing the liquid MenB component of Penmenvy to a patient. They asked whether it should have been reconstituted with another dose of Bexsero to provide a full dose.
