medication was mixed with sterile water instead of the adjuvant; medication was mixed with sterile water instead of the adjuvant; This non-serious case was reported by a other health professional via call center representative and described the occurrence of wrong solution used in drug reconstitution in a 55-year-old female patient who received Herpes zoster (Shingrix) solution for injection (batch number 4m377 with expiry date 26-AUG-2027) for prophylaxis. On 29-MAY-2026, the patient received the 1st dose of Shingrix. On 29-MAY-2026, an unknown time after receiving Shingrix, the patient experienced wrong solution used in drug reconstitution (Verbatim: medication was mixed with sterile water instead of the adjuvant) and inappropriate dose of vaccine administered (Verbatim: medication was mixed with sterile water instead of the adjuvant). The outcome of the wrong solution used in drug reconstitution and inappropriate dose of vaccine administered were not applicable.
