Symptom narrative

Patient received the above listed vaccines at approximately 10:00am. He was in stable condition upon leaving the school clinic. No issues or complaints reported during the rest of the school day. Upon arriving home, the patient reported difficulty breathing to his father who took him to the emergency room. He arrived at approximately 5:00pm and dyspnea reported as patient's chief complaint. His O2 saturation was in the low 70%. He was given oxygen and diphenhydramine, epinephrine, and methylprednisolone was administered around 5:30pm. His O2 sat improved to >94% on room air at 6:35 pm. He was discharged to home at 7:56pm in stable condition.
