Symptom narrative

Staff member with RN degree mistakenly administered Pentacel with the intention of administering DTaP. That staff member immediately performed the ordering provider who then spoke with parent/guardian of patient. That staff member was then scheduled to meet Risk Department. The parent/guardian reported no signs/symptoms following the vaccination.
