Symptom narrative

Patient received 3 vaccines on 04/22/2026. Menveo and Gardasil was patient's first time getting these vaccines. Boostrix Tdap 2.5 Lf unit-8 mcg-5 Lf/0.5 mL intramuscular syringe Menveo A-C-Y-W-135-Dip (PF) 10 mcg-5 mcg/0.5 mL IM solution (1 vial) Gardasil 9 (PF) 0.5 mL intramuscular syringe Patient received vaccination of Gardasil in the left deltoid. Patient received Menveo and Boostrix in the right deltoid. After all vaccines were given patient guardian was told to wait 15mins before leaving. It was about 5mins and patient guardian stated patient was having ringing in ears and felt nauseous. I had patient lay back slightly on the bed and try to relax. I quickly informed PCP. PCP went into room and patient vomited. While PCP was in room I went to grab crackers, water, and juice. BP was 98/66, O2 98, and Pulse was 73. PCP was aware of current vitals. Vomit was cleaned up with a spill kit. Patient's vitals were monitored throughout visit and blood sugar was checked and read 107. Patient started feeling better and was able to stand. Before leaving office, patient did do vision exam for a sports physical. Patient was 20/20 in both eyes.

Allergies

cetirizine 5 mg/5 mL oral solution Take 5 mL every day by oral route as needed for 90 days. Tylenol 160 mg/5 mL oral suspension Take 15 mL every 6-8 hours by oral route as needed. May alternate with ibuprofen every 4 hours ibuprofen 100 mg/5 mL oral suspension Take 15 mL every 6-8 hours by oral route as needed. Note to patient: May alternate with Tylenol every 4 hours Cetirizine 5 mg chewable tablet Chew 1 tablet every day by oral route as directed for 30 days.
