Symptom narrative

Pt came back to the pharmacy. She said she noticed some red rash like mark on the arm of the injection the night of the 15th and there was still some today. This was her second injection. She said at first there was some pain but this morning slight itching. She said the symptoms were mild. She said the night of the vaccine she had some itching and chills but this was expected for the 2nd dose.
