HTN, allergies, GERD, GAD and dry eyes.

Other medications

Pravastatin, HCTZ, Metoprolol Succinate, Montelukast, vitamin D, Lumify drops, hair & nail vitamin, Tumeric cap, Aspirin, Protonix, Duloxetine and Loratadine.

Allergies

Aloe vera, latex, e-mycin and sulfa drugs.

Lab data

Lipase, POC Chem 8, CBC w/diff, hepatic function panel, CXR, OV-2/Flu combo PCR and UA.

Prior vaccinations

Shingles, same symptoms, age 56, received first dose on 4/19/2022 manuf. by GlaxoSmithKline.
