Symptom narrative

At 10:15am, patient was given Boostrix in left arm followed by Gardasil 9 approximately 1 inch below. Immediately after administration, patient reported to feel dizzy. Patient proceeds to faint in her chair and present seizure-like convulsions that last about 1-2 minutes. When patient woke up, she was able to say "I think I just blacked out". Patient felt very nauseous and vomited. Blood pressure was taken and reported to be 98/69 and HR 82. EMS was called and she was taken to the emergency room. Patient was sitting in her chair the whole time and did not fall or hit her head.
